To the uninitiated, this little ditty is daunting, even if your unit is lucky enough to have an assistant travel planner to boot. But it does not have to be this way. A slick online system is all it takes to get the swag done in a snap. This is the reason why I have devoted this article to DD Form 1352, as well as its lesser-known counterparts.

The Department of Defense (DOD) is no slouch when it comes to travel and entertainment, and they are not shy about letting their personnel spliff the pants. However, all the fun and games is at stake if you are not properly reimbursed for your hard-earned dollar. You’re not the only one, as the military is no stranger to financial hiccups, but the industry’s best practices have helped curb the tide.

If you are a Service member or civilian government employee, you need to know the Instructions for DD Form 1352. These instructions cover all types of Service member discharges, including death, separation, retirement, and other events. The instructions also apply to the retirement of Service members and their dependents. For these instances, the service must make complete STRs available to the VA within 45 calendar days after a Service member separates. This process is called STR disposition.

STRs are used to provide an official record of medical care and are maintained by MTFs. They are hand-carried in a sealed envelope. Each page of the envelope is annotated with the patient’s demographic data. When a patient has allergies, this information should be visible. In addition, the SF 600 is a chronological record of medical care. SF 600s from single-visit encounters must be filed in a chronological order.

Documents generated by deployed locations include inpatient discharge summaries, narrative summaries, and operative reports. Those documents will be uploaded into AHLTA-T. Deployed locations will file AF Form 3899, which is a medical records summary, in DD Form 2766.

The sponsor will be notified when an individual receives direct health care at a different MTF. The acquiring MTF will request outpatient records through PRT and cover the GSU label with a blank one. Once the incoming records are received, they will be filed according to the instructions in Appendix 1 to Enclosure 3. This system will index the records to the appropriate STR parts.

TRAVEL VOUCHER OR SUBVOUCHER

A Travel Voucher or Subvoucher is a form that needs to be filled out by military members who wish to receive compensation for their travel expenses. This form is also known as DD Form 1351-2 and is required by the Department of Defense. You must fill out this form and submit it within thirty days of your travel. It is also very important that you include all of your trip details on the form. Make sure to include the dates of your trip, the place you went to, and the time you spent there. Also, include the cost of your ticket and your Commercial Card transaction number. You should include as much information as possible on the form, as this will increase your chances of being reimbursed.

When completing the form, you should also include any supporting documents that you have. These should be printed out on one sheet of paper and attached on the same side. They should not hang over the edges or overlap with the data.
